In this document you will be informed about the services we provide, confidentiality (and the limitations thereof), and my background. HPCMHC offers various services that include: substance abuse counseling, psychiatric evaluation, individual counseling, group counseling, peer support groups, and medication monitoring. All residents of Pima county are eligible for services. As a client of HPCMHC, you have been required to obtain counseling sessions by a judge who is overseeing your case. As your SAT I am required to report the following to your probation officer: Missed appointments (without prior arrangements), missed payments, and an assessment of whether counseling sessions are benefiting you. Due to the nature of your referral it is important that you understand that I may be required to disclose to your probation officer anything that you say to me in our sessions.
